your privacy. This document describes how Vermont
Center for Human Growth and Development may
use and disclose information about you and how you can access this
information. If you have any questions about this notice please

Client
Information
Client
files are held securely and are accessible only to authorized employees
of the practice. The information in each file includes personal
information such as name, address, contact phone numbers and other
information relevant to the service being provided.
You have the right to review and obtain a copy of health information
that may be used to make decisions about your care. This may include
both psychotherapy and billing records.
Release of confidential information
All personal information gathered by your psychotherapist will remain
confidential and secure except when:
1) it is subpoenaed by a court
2) failure to disclose the information would place you or another
person at risk of harm.
3) your prior consent has been obtained in writing to communicate
with another professional or agency.
Concerns
or Complaints
If
you have a concern about how your personal information has been
managed, please discuss your concerns with Diana Greywolf. You may
file a formal complaint with the Vermont Secretary of State
Office of Professional Regulation, 81 River Street, Montpelier,
VT 05609-1104.
Requests for access to client information
At
any stage clients may request to see the information about them
kept on file. The psychotherapist may discuss the contents with
them.
Purpose
of holding information
The
information is gathered as part of the assessment, diagnosis, and
treatment of the client's condition and is seen only by authorized
employees of the practice. The information is retained in order
to document what happens during sessions, and enables the psychotherapist
to provide a relevant and informed service.
This notice was published and becomes effective on
January 16, 2007.
